    How do I apply?
Application forms are available from the Lymphoedema Compression Garment Program, lymphoedema trained therapists and by clicking on the application form logo below. The form needs to be completed by you, the medical practitioner who diagnoses lymphoedema and the qualified lymphoedema therapist providing treatment and prescribing garments. The form includes a declaration of income which needs to be completed with a willingness to supply any further financial information if necessary, particularly regarding financial difficulties. Information provided by applicants is kept confidential. Applicants need to sign a privacy statement to protect their privacy.

Note: In order to prescribe garments, lymphoedema therapists must be eligible for membership of the National Lymphoedema Practitioners Register.

The financial assessment is made on the basis of your income and the cost of the garments that have been recommended for you. Other factors such as financial difficulties are also considered. Applications are processed within two weeks. You and your lymphoedema therapist will be notified of the result. If you do not agree with the result of your assessment and would like to appeal, please contact the Co-ordinator of the Lymphoedema Compression Garment Program on 03 8458 4981.
Your therapist will discuss with you the most appropriate garment for your needs. A referral is made to an approved supplier who will need your financial contribution when the garment is ordered.

Suppliers are approved by the program to supply compression garments that conform to quality standards. The program will be invoiced to pay the remainder of the cost and the garment will be sent to you or your therapist.

The program will provide funding for up to six garments per year, which is facilitated by your therapist.
