Your treating team can only provide the appropriate care if they know all the relevant information regarding the health and circumstances of you and your family.

You have a responsibility to give the healthcare professional accurate information when asked about your health, including any current medication.

You have a responsibility to tell the healthcare professional, as soon as possible, if your health changes.

You have the responsibility to ask for an interpreter if you need one.

You have the responsibility to ask the healthcare professional to explain any part of your treatment that you do not understand.

You have a responsibility to be considerate of staff and other clients and to ask your visitors to be considerate.
