Mercy Health O’Connell Family Centre can receive referrals from health professionals and community providers.

Health professionals and community providers can send referral forms directly to Mercy Health O’Connell Family Centre.
 

Referral Procedure
Once the referral has been made, our triage nurse will liaise with the provider and the potential client about the suitability of our services for their needs. A pre-admission consultation is scheduled onsite or over the phone, depending on the location of the family.

If our services are suitable, the family will meet our staff and have a tour of our facility. Outcomes for the family will be discussed and discharge planning will begin. A discharge letter will then be sent and there will be further contact after discharge as needed.

Our team can be in contact with the relevant professionals during the family’s stay, providing progress updates as required. Case meetings can be held with invited participants.
