STEP 4: The application process

  • Once you have completed your ACAS and your eligibility for an Australian Government subsidy has been determined, you will be ready to apply for residency.

    Although application packages are available on various government and aged and community care websites, Mercy Health has tailored a comprehensive Residential Care Application form. Click the Application Form to download a form or contact your preferred Mercy Health facility .

    Whether you are applying for permanent residential aged care or respite, you will need to give your completed Mercy Health application form and ACAS assessment documents to the residence of your choice for consideration.

    The Mercy Health application form asks you to provide the following information, which remains confidential at all times:
    • Personal and secondary contact details
    • Enduring and medical powers of attorney
    • Care and medical history
    • Financial assessment details

    Mercy Health offers places when vacancies arise and you are welcome to apply to more than one facility. If there are no places available at the time of your selection, you can ask to be placed on a waiting list. If personal circumstances change after the date of your application, you are not obliged to accept a placement offer.

    If you accept an invitation to move into your new Mercy Health home, you will need to meet with the Service Manager or Resident Liaison Officer to complete a residential agreement before you move in. You may also be asked to present a medication chart, which will assist staff in drawing up your care plan. (You can ask your general or medical practitioner for a medication charts.) Once your agreement has been authorised, you can make the necessary arrangements for the next stage of moving into your new home.
